The PCP project INCAREHEART “Pre-commercial Procurement of innovative ICT-enabled integrated care solutions to advance multidisciplinary health and care for patients with chronic heart failure” will introduce seamless transitional care processes effectively reducing emergency care needs, hospitalisation rates as well as increasing treatment adherence.

In recent months, empirica together with the INCAREHEART buyers group as well as other organisations (TicBiomed, IFIC) have finished a series of OMC (Open Market Consultation) events in five regions: Jämtland, Turkey, Naples, Thessaloniki, and Amadora. OMC is a crucial milestone in the entire PCP process to cultivate interest and the same time tap into industry-wide expertise.
