Challenge

Patients often struggle to access and understand trustworthy, relevant medical information tailored to their needs. Traditional sources such as printed leaflets are often too technical or disconnected from the patient’s individual health context, contributing to misinformed decisions and reduced safety in medicine use. Digital health information tools often not patient-centric, leaving a gap in support for self-care, informed consent, and optimal treatment journeys.

Solutions

Gravitate-Health aims to empower citizens by co-developing and demonstrating the Gravitate Lens (G-Lens), an open-source, digital health information platform that filters and presents regulator-approved content (e.g., ePI – electronic Product Information) and relevant health records (e.g., International Patient Summary) in a user-centric, context-aware way. Patients, their caregivers, and health professionals will be able to access focused and tailored information that enhances safety, understanding, and engagement. Gravitate-Health provides personalized services such as reminders, contraindication warnings, and simplified medication insights, fostering two-way communication and better support throughout the patient journey.

Results

Gravitate Health improves health information understanding and safety through personalized risk minimization strategies. It empowers patients through self-care tools and better system navigation by providing an open-source platform that can be reused and scaled across health systems.
